When i apply your default promotion (Shirts Special) if i choose a default sku it works but if i choose a shirt with custom sale price on a sku (e.g size M and color black with a custom sale price), the promotion is not applied : http://imgur.com/QjA5g

There actually isn't a default sku for shirts. What's happening is a restriction that the promotion is only applicable once per cart.

For example, if you only have that shirt in Black/Medium, it will have a promotion:

You can configure the maximum uses per promotion in the admin.
